To schedule an appointment with one of our offices, please call the location of choice and speak to one of our friendly representatives.

On the day of your appointment please sure to bring the required documents:

Photo ID
Insurance Card
List of medications

* If it is your first time at PrimeNet Medical please be sure to arrive 15 minutes early to fill out the appropriate paperwork.

** If you are part of an HMO Medicare plan or a Medicare beneficiary and are in need of transportation, please notify our office at the moment of scheduling.

*** If canceling an appointment please notify the office 24 hours in advance.
